HEALTH CARE PLAN

NCH sponsors a self-funded health care plan to help cover the costs of medical care. Full-time and regular part-time employees are eligible to participate in this coverage after certain waiting periods or qualifying events. Both the employee and North Country Hospital contribute to the premiums for the plan. Our health care plan features the opportunity to add additional dollars to your Health Reimbursement Account by participating in healthy lifestyle activities.

DENTAL INSURANCE

Employees have the option to choose dental coverage. The plan is a comprehensive preventive care plan.

VISION CARE PLAN

Full-time and regular part-time employees are eligible to participate in this coverage after certain waiting periods or qualifying events.

PAID TIME OFF

Full and part-time employees are eligible to accrue paid time off from the first hour worked and may begin to use their PTO benefit upon successful completion of their 90- day Introductory Employment Period. The amount of PTO is based upon hours worked in each pay period up to a maximum of 80.

EDUCATIONAL ASSISTANCE

North Country Hospital offers several educational assistance programs that support our employees in achieving their professional and personal educational goals.

SHORT-TERM DISABILITY

NCH provides coverage to full and regular part-time employees after 90 days of employment. This benefit covers you if you if you suffer an illness or accident for up to 13 weeks and is provided to you by the hospital at no cost to you.

LONG-TERM DISABILITY

Full and regular part-time employees will be eligible for coverage after 90 days of employment. Benefits of up to 60% of salary begin after 90 days of covered illness or injury.

RETIREMENT PLAN

North Country Hospital offers to all eligible employees to opportunity to participate in its 403(b) Tax Sheltered Annuity plan. This plan is an important element in providing employees the potential for future financial security for retirement. Under this plan you are able to direct a percentage of your salary into investment options. North Country Hospital will match a percentage of your voluntary contribution to this plan. This provides you with the opportunity to tailor your retirement package to meet your individual needs. Both pre-tax and post- tax ROTH options are available.

WELLNESS CENTER PROGRAMS

Employees are eligible for discounted rates for Wellness Center courses.

FITNESS CENTER MEMBERSHIP REIMBURSEMENT

NCH encourages employees to achieve and maintain a healthy lifestyle through physical fitness. Full and Part time employees are eligible for reimbursement of membership at a fitness center.

FIT & FUN CLUB
