North Country Surgical Associates, located at 41 Medical Village Drive on the North Country Hospital Campus is here for all of your general and vascular surgery needs. Our office is open Monday-Friday from 7am-4:30 pm
We also provide cosmetic procedures for treatment of Varicose Veins and Permanent Hair removal using our state of the art Laser Machine. We also provide Botox and Juvederm treatments in our office.

Managed Care (HMO) Medicare and Medicaid patients may need a referral from their primary care provider before scheduling an appointment.

We have 24 hour call, after hours messages can be left on our answering machine and we will return you call at the next business day or follow instruction to reach the surgeon on call.

Appointment
Patients can refer themselves to our office or be referred by their primary care physician depending on their insurance coverage. We try to see all patients on an appointment basis and we request that you call in advance so we can reserve time for you.

We make every effort to honor all time commitments and request that you extend the same courtesy to us. On occasion, emergencies can arise and whenever possible, you will be fully informed if there will be any delays because of this. If you cannot keep your scheduled appointment, please give 24 hours notice whenever possible.

Emergency Care
We recognize that an emergency situation can arise and we will do our best to respond to your problems promptly. In the event of a severe situation, go immediately to the emergency room and ask them to contact your doctor. If you are in doubt or the situation is not that severe, but you wish to contact your doctor, please telephone the office and the doctor will be in touch with you either directly or through office personnel to give instructions.

Telephone Calls
Patients are encouraged to call with questions they have regarding their surgery or recovery. The office staff has been trained to answer many of your questions. They will also relay your information to the doctor and your call will be returned at the earliest opportunity.

After Hours Telephone Calls
If you find it necessary to contact the doctor after normal office hours, our answering service will forward all messages to your doctor or the doctor on call. Your call will be returned at the earliest opportunity.

Prescriptions and Renewals
All prescriptions and authorizations for renewals should be requested during normal office hours. Please do not wait until you are completely out of your medication to notify us.

Fees and Payments
We make every effort to control the cost of your medical care. Payment is due at the time services are rendered. If you are scheduled for surgery and your insurance does not cover all charges, other arrangements can be made by speaking with someone in the credit office at North Country Hospital. If you do not have insurance, we will gladly set up a payment plan to assist you.

Balances are due and payable within 30 days of billing. Statements are sent out monthly and if balances are paid in full, we offer a discount based on the amount due. If you have questions regarding your bill, please direct them to Central Billing or the Credit office at North Country Hospital.

We are part of the North Country Health Systems therefore any arrangement for payments must be set up with the Credit Department.

Insurance
We will gladly file your insurance claims for you. Please be sure we have updated insurance information and provide us with a copy of your insurance card.

If you are seeing us for a Worker’s Compensation injury, please be sure we have all the necessary information to bill that carrier.

Health insurance is intended to cover some, but not all of the cost of your treatment. Most plans include co-insurance, deductible and other expenses, which must be paid by the patient.

Some insurance companies do require pre-certification for surgical procedures and pre-admission review. Our staff is prepared to call the insurance carrier to take care of this for you.

Remember that an insurance plan is a contract between the insurance company and you, the patient. While we will assist you in any way that we can to obtain the maximum benefits from your policy, you are ultimately responsible for payment for the services rendered you by our office.

Smoking is prohibited in any part of our building. Please honor this regulation.

Cosmetic Procedures
We are please to announce that we now provide some cosmetic laser procedures for permanent hair removal, varicose veins, spider veins, Botox and Restylane/Perlane injections. For more information please call our office.
