Our 2 Primary Care Locations, Newport and Barton, are both nationally recognized Patient Centered Medical Homes. We are a 20 Provider multispecialty practice with both Family Medicine and Internal Medicine that is staffed with both Physicians and Midlevel’s (PA’s and NP’s) covering internal medicine, family medicine, psychiatry and geriatrics. Our Pt Care team includes our Nurses (RN’s + LPN’s), Care Coordinators (RN’s), Dieticians and Social Workers as well as our Primary Care Director(RN), Clinical Manager(RN) ,Medical Group Educator (RN),who are supported by our Administrative Manager and Administrative staff.
